- Check your record on ISIS regularly to ensure there are no holds.
- Update your address with the University’s Registrars Office to ensure you receive your Diploma.
- Mailing Address after your last Internship for your Student File. Complete and return via e-mail the attached Address Form to mjodom@phhp.ufl.edu.
- Update your Emergency Contact in ISIS also.
- Order your Commencement Regalia: see commencement section below.
If you were recently married or will be changing your name as you seek licensure and certification, please notify the University’s Registrars Office to officially change your name with the University. Please also e-mail us with any change as well at: mjodom@phhp.ufl.edu.
The names on all correspondence from the OT department to NBCOT and to State licensure boards must match with your name in the University’s Registrars Office, and must match the name on your diploma and transcripts.
MOT Department Graduation Reception and Awards Ceremony
This event will be held in the HPNP Reception Area, located on the ground floor, west side, at 10:45 AM, Friday, December 13th. The Awards Ceremony will follow the reception at 11:00AM to 12:00 Noon, in the HPNP Auditorium. Please invite your immediate family to attend. Dress is business dress.
Upon arrival to the reception, please pick up your name card for the announcer, so that you can be recognized by name for your achievements as you walk across the stage.

Ordering Regalia
The deadline for ordering academic regalia is Friday, October 25, 2013. Information on ordering regalia is available from the UF Bookstore (392-0194) or the bookstore website: http://www.bsd.ufl.edu/G1C/bookstore/graduation.asp
Masters Commencement Ceremony
Saturday, December 14, 2:00 PM. All candidates must assemble at 12:30 p.m. at Stephen C. O’Connell Center. The Processional begins at 1:30 p.m.

Certification Exam and Licensure
Review your Blue Folder for important information. A letter confirming your completion of the program will be automatically mailed to the State of Florida MQA after we receive your evaluation from your final internship. If you are applying for licensure in a different state, please e-mail or mail the required verification documents to mjodom@phhp.ufl.edu.
Medical Errors Course
A medical errors is required for Florida licensure. The Role of Occupational Therapy in Patient Safety and Medical Error Reduction is approved to meet this requirement and is available online via UF Department of OT at http://ceu.phhp.ufl.edu/. The cost is $25 and you may pay with a credit card. A certificate of completion will be emailed to you as a PDF at the email address you provide on the course website.
