- Check your record on ISIS regularly to ensure there are no holds.
- Update your address with the University’s Registrars Office to ensure you receive your Diploma.
- Mailing Address after your last Internship for your Student File. Complete and return via e-mail the attached Address Form to mjodom@phhp.ufl.edu.
- Update your Emergency Contact in ISIS also.
If you were recently married or will be changing your name as you seek licensure and certification, please notify the University’s Registrars Office to officially change your name with the University. Please also e-mail us with any change as well at: mjodom@phhp.ufl.edu.
MOT Department Graduation Reception and Awards Ceremony
This event will be held in the HPNP Reception Area, located on the ground floor, west side, at 3:00 PM, Friday, December 18th. The Awards Ceremony will follow the reception at 3:30 PM to 4:30 PM, in the HPNP Auditorium. Please invite your immediate family to attend. Dress is business dress.
Upon arrival to the reception, please pick up your name card for the announcer, so that you can be recognized by name for your achievements as you walk across the stage.

Masters Commencement Ceremony
Saturday, December 19th, 2:00 PM. Assembly and Processional at Stephen C. O’Connell Center.
Candidates assemble at 12:30 p.m.
Processional begins at 1:30 p.m.

Certification Exam and Licensure
Review your Orange Fieldwork Folder for important information. A letter confirming your completion of the program will be automatically mailed to the State of Florida MQA after we receive your evaluation from your final internship. If you are applying for licensure in a different state, check the state’s website and then e-mail or mail the required verification documents, after you complete your portion, to mjodom@phhp.ufl.edu.
Medical Errors Course
A medical errors is required for Florida licensure. The Role of Occupational Therapy in Patient Safety and Medical Error Reduction is approved to meet this requirement and is available online via UF Department of OT at http://ceu.phhp.ufl.edu/. The cost is $25 and you may pay with a credit card. A certificate of completion will be emailed to you as a PDF at the email address you provide on the course website.
